find some one who knows a account manager or some one good working their.
Also Read their terms and conditions about accepting people, they probly wont accept a myspace site as it might not be in the acceptable sites guide lines.

I did have them but got dropped, they seem to have dropped about 99% of myspace sites... it really isn't that great anyways, their fill rates were terrible for me

Many CPA type networks require a quick phone interview if they are interested now. One good way to make sure you get at least that is to say you are also planning PPC marketing and such as well. They may not want to approve you if all you plan to do is add banners on your myspace site but if you tend to do, or show you plan to, some affiliate marketing with PPC and such they are more likely to approve you.
You can tell them that you plan to put banners on your existing sites but also plan to start up some PPC campaigns as well.
Of course it helps if you mean it and follow through but many of the networks prefer only working with people who will definitely drive traffic to their offers.

I've applied to Casale Media several times and have always been rejected. I'm hearing ValueClick is the way to go though?

I got into Casale a few weeks ago. Depends on the site, content, and how many ads you are already running.

I got into Casale about two weeks ago. At that time, I was only running about 3 Bidvertiser ads. I agree with nsmchris, the ads you have currently running on your site is definitely a factor.

I've been rejected by them once before when I applied, I would love to get in but how well do they pay?

their pay is decent, comparable with ValueClick but I've heard they have low fill rates (a few people have reported only a 30% fill rate) and you will definitely want to have defaults set up.
